A recent study published in the journalPharmacology of Particle Therapyindicates that the use of sildenafil, the active ingredient in Viagra, is associated with an increase in blood pressure in the arteries of the legs of people with angina or other conditions in general. Sildenafil, however, is not associated with an increased risk for stroke, coronary artery disease, or myocardial infarction, although these events may be prevented by using a safer, lower dose. Sildenafil is also found in the blood plasma of menopausal women and in women who have or have ever had breast cancer. Sildenafil is not known to have an effect on other medications that might cause the drug to be associated with an increased risk for an adverse reaction.
The study was published in the journal. It was an observational, case-control study of 269 cases and 269 controls from five large, non-randomized, multicenter clinical trials. It used data from the FDA’s Clinical Event Data System (CEDS) as the source for data on events.
The control group consisted of 269 individuals with either no risk factors or in whom a single drug had been associated with an increased risk of cardiovascular events. Individuals who were found to be at an increased risk for an adverse reaction included those with (N = 60) and without (N = 64) cardiovascular events. Of the 269 controls, 277 (56.5%) had had cardiovascular events before taking sildenafil, of the 269 men, 62.5% had had coronary artery disease, while only 13.3% had had an adverse reaction to any of the drugs used in the study. Of the 269 controls, 74 (8.6%) had cardiovascular events before taking sildenafil. Of the 269 men, 67 (11.9%) had coronary artery disease and 20 (7.6%) had had an adverse reaction to sildenafil. Of the 269 men with a positive CEDS result for an adverse reaction, the most common events were angina (13.3%), coronary artery bypass graft (CABG) (11.1%), stroke (9.9%), and myocardial infarction (MI) (10.1%).
Sildenafil was associated with an increased risk for major adverse cardiovascular events (myocardial infarction [MI]) in an observational, case-control study of the European Prospective and Cohort Study 2, which included 304,842 men over 40 years of age. The men in the study were aged 50 to 55 years (mean age 66 years), and the study included 906,092 men with a mean duration of follow-up of 8.5 years. Sildenafil use was associated with an increased risk for a type of cardiovascular events (myocardial infarction or stroke) in men aged 50 to 64 years, 55 to 64 years, or men with a history of MI or stroke. The hazard ratio for the men in the study who used sildenafil compared to placebo was 1.6 (95% confidence interval [CI] 1.1 to 2.5). The men who were found to have an increased risk for a type of cardiovascular event in the placebo group also had an increased risk for a cardiovascular event. The study did not find an increased risk for all events.
The data was analyzed using the Cox proportional hazards model, which considers the potential confounding by smoking, hypertension, and diabetes as confounding variables. Cox proportional hazards models have a small amount of variation (mean [SD] 1.8, 95% CI, 0.9 to 1.2) and so are not useful in the study of the effects of drugs on a person’s health. Because the study was not designed to assess the effects of sildenafil on cardiovascular events, the results of the Cox model can be interpreted as a “statistically significant” result. However, the results of the Cox model do not exclude the possibility that other drugs may have contributed to the increased risk of cardiovascular events. If the results of the Cox model are interpreted as astatistically significant result, the data of the study could be interpreted as evidence that sildenafil may be associated with an increased risk for a cardiovascular event.
The results of the study do not show any association between the use of sildenafil and an increased risk for any cardiovascular event.
